Upper Endoscopy (EGD) Before Bariatric Surgery
Why EGD is recommended, what it looks for, and H. pylori treatment before surgery.
As part of your bariatric surgery evaluation, your surgeon may recommend an upper endoscopy, also called an esophagogastroduodenoscopy (EGD). An EGD allows your surgeon to examine the inside of your esophagus, stomach, and first part of the small intestine using a thin flexible camera.
What Does an EGD Look For?
- Inflammation of the esophagus (esophagitis) or acid reflux (GERD)
- Hiatal hernia, stomach ulcers, or gastritis
- Helicobacter pylori (H. pylori) infection
- Barrett's esophagus or stomach polyps
- Rare stomach or esophageal tumors
Why Is EGD Important Before Bariatric Surgery?
Many patients have stomach or esophageal conditions without realizing it. Severe acid reflux or Barrett's esophagus may influence whether gastric sleeve or gastric bypass is the better operation. Treating stomach ulcers or H. pylori infection before surgery may also reduce the risk of postoperative complications.
Is an EGD Painful?
No. The procedure is usually performed under sedation, and most patients do not remember the examination. An EGD generally takes 10 to 20 minutes, and patients typically go home the same day after recovering from sedation.
H. pylori Before Bariatric Surgery
What Is H. pylori?
Helicobacter pylori (H. pylori) is a type of bacteria that lives in the stomach. Many people have no symptoms, while others may develop chronic gastritis, peptic ulcers, indigestion, or stomach pain. Because many patients have no symptoms, H. pylori is often discovered only after a biopsy taken during an EGD.
Treating H. pylori before bariatric surgery may reduce the risk of future stomach ulcers and other complications—especially important before gastric bypass, because the bypassed portion of the stomach becomes difficult to examine with routine endoscopy after surgery.
- Treatment typically includes antibiotics and a proton pump inhibitor
- Treatment usually lasts 10–14 days
- Confirmation of eradication may be recommended before surgery
